This is a contractual agreement with Five Star Management, LLC (hereafter referred to as 5 Star). The purpose of this Agreement is to outline the mutual obligations between Client and 5 Star. 5 Star promises to hold and make available to Client seminars, notes and materials of the seminars each year for three years, plus any other expressly written privilege, service or information.

Client agrees to make thirty-six (36) consecutive monthly payments of $1,200.00 USD per month due on or before the 1st of each month starting on the date of this Agreement.

At Client's option, Client may discontinue 5 Star in the 12th month of this Agreement after making twelve (12) monthly payments of $1,200.00 USD (for a total of $14,400.00 USD) and an additional payment of $4,800.00 USD, for a total payment amount of $19,200.00 USD.

This Agreement is binding on both parties until the contractual agreement is fulfilled. Two (2) late payments may, at 5 Star's option, accelerate the entire remaining unpaid balance of the Agreement, making the remaining balance due and payable
immediately. The terms, conditions and any payments called for under this Agreement shall be subject to the jurisdiction and venue of the Superior Court of King County, Washington State. In the event that 5 Star is required to take legal action in connection with this Agreement and 5 Star prevails, 5 Star shall be entitled to what are adjudged as reasonable attorney's fees.

I give 5 Star permission to collect payment through monthly and/or lump sum credit card charges.
