[00:00:00] Right now, I mean literally right this second, within a three-mile radius of your clinic, there are dozens of people holding their phones. Oh, absolutely. And they are in active pain. You know? They are desperate for relief, and they are ready to spend money to feel better today. Right. They want a solution right now.
Exactly. And if you aren’t dominating the top three inches of their screen when they search for help, well, you just don’t exist. You can give the absolute greatest adjustments in the world, but without visibility, um- Empty tables don’t heal anybody They really don’t. It is the ultimate bottleneck for any practice because being the best-kept secret in town, well, that doesn’t pay the overhead.
No, no, no, it doesn’t. And it certainly doesn’t fulfill the mission of getting sick people well. I mean, visibility is what bridges the gap between your clinical skills and the community that actually needs them. Well said. Okay. Welcome to the Chiropractic Deep Dive. It is a part of the Successful Chiro podcast.
This deep dive is brought to you by Five Star Management, a [00:01:00] chiropractic consulting company. Glad to be here. Today, we are unpacking a really incredible masterclass of a Zoom session. This was led by Dr. Noel Lloyd, and it featured digital marketing expert Nick Fischer. Yeah, that was a fantastic session. It really was, and the sole focus of it was search domination, so we’re looking at exactly how chiropractors can ensure their practice is the number one recommended resource, um, when patients are actively searching for help.
So to really understand search domination today, we have to look at the three major pillars that dictate whether you show up or, you know, stay completely hidden. Right. Big three. Exactly. Relevancy, distance, and prominence. So relevancy is the mechanical match. Does your online footprint actually align with the specific symptom the patient just typed in?
Makes sense. Then distance is your physical proximity to their phone, which, I mean, you can’t really change unless you physically move your clinic. Right. You are where you are. But prominence, that’s about how well-known, trusted, and verified you are in the [00:02:00] digital space. That is the lever you can actually pull.
That is where the game is won or lost. It really is. Yeah. So our roadmap for today covers the entire journey of prospective patients scrolling on their phone. We’re exploring paid ads, the map pack, and organic website rankings. A full sweep. Yeah, exactly. But before we get into the long-term organic strategies, which, let’s be honest, take months to build momentum, we need to talk about cash flow today.
Crucial. Because if a clinic is sitting at, say, 50 visits a week and they need to be at 150, they cannot wait a year for SEO to kick in. They need the phone ringing on Monday morning. Which brings us to what Nick Fischer calls the cheat code of search, which is paid placements. The cheat code. I like that.
It really is though. I mean, you can build a campaign on a Tuesday morning and literally be the first thing a patient sees by Tuesday afternoon. Wow. But how you spend that budget dictates whether you get a flood of new patients or, you know, just a, a totally drained bank account. The source material draws a very hard line [00:03:00] between two distinct types of paid ads.
Right. Traditional pay-per-click, or PPC, and local service ads, known as LSA. Yeah, and that distinction is completely foundational. With PPC, you are paying every single time someone clicks your link, regardless of what they do next. Which can be risky. Very. But with local service ads, you are paying per phone call.
Paying per phone call. Yeah. That is huge. It’s like instead of casting a fishing net with giant holes where all the fish just swim right through, you are actually getting a fish on the line every time you pay. That’s a great analogy. Let’s look at the psychology of that for a second. Right. A click to a website is incredibly low commitment.
The user might just be browsing or, um, maybe they get a text message and close the window. Or they get distracted. Exactly. But a phone call, that is a high friction, high intent action. It puts that prospective patient directly on the line with your front desk. And Google actually requires a background check for those, right?
They do. To run an LSA, you have to go through a [00:04:00] verification process to get the Google guaranteed badge. Oh. So when a patient sees that badge, they instantly trust you more. Yes, an LSA lead costs more upfront than a standard website click, but the actual conversion rate into a booked appointment is astronomically higher.
You are buying a live conversation, not just digital foot traffic. That makes total sense. Now, if a clinic is running traditional PPC, the source material hammers home the absolute necessity of relevant landing pages. Yes. You have to get this right. And I’ve seen this mistake constantly. Like a clinic runs a beautiful ad for spinal decompression.
A patient with three herniated discs clicks it. But instead of seeing a page about decompression, they get dumped onto the clinic’s generic homepage. The one that just talks about pediatric care and general wellness. Exactly. And the patient immediately experiences, well, cognitive friction. They were promised a specific solution for severe pain, and instead they have to go hunting through a navigation menu to find it.
And they won’t do it. They just hit the [00:05:00] back button. Right. And here’s the mechanical consequence of that Google tracks that bounce. If a high percentage of people click your ad and immediately bounce back to the search page, Google basically concludes your ad is misleading. That’s fascinating. Yeah. They will actually start charging you more per click and showing your ad less frequently because you are providing a bad user experience.
Because Google protects the user experience above all else. 100%. And another vital piece of optimizing that ad budget is ad scheduling. I mean, if you do not have an unlimited budget, you really shouldn’t be running ads at 2:00 AM on a Sunday when your clinic is completely dark. No, that’s just burning cash.
Right. You schedule those ads to run precisely during the hours your front desk team is actually sitting at the desk ready to answer the phone and capture that high intent lead. It sounds so simple, but an alarming number of clinics burn 20% of their monthly ad spend on clicks that happen when nobody is around to actually pick up the phone.
That’s crazy. Now, in the [00:06:00] Zoom session, this concept of negative keywords was highlighted as a major way clinics bleed marketing dollars. Oh, yeah. It’s a huge issue. But I need to push back on this a bit because I think the logic can be confusing for a lot of people. Okay, let’s hear it. If I tell Google to block my ads from showing when someone types the word salary or jobs, aren’t I risking filtering out, say, high income earners in my town who might be searching for salary information in their own industry but also happen to have back pain?
Like, how smart is this net? That is a very common concern, but it basically misunderstands how the algorithm applies the negative keyword. You aren’t blocking the user based on their demographic or their search history. Oh, okay. You are blocking the specific search query in that exact moment. Let’s say you bid broadly on the word chiropractor.
Google might show your ad to someone actively typing chiropractic assistant jobs near me Ah, I see That user clicks your ad looking for a job application. You just paid [00:07:00] three dollars for that click, and it is entirely useless to your practice Right. They aren’t a patient Exactly. So negative keywords act as guardrails for that specific search moment.
You tell Google, “Do not show my ad if the phrase being typed right now includes the words jobs, salary, school, or my direct competitor’s name.” It instantly stops the bleed of irrelevant clicks Okay, that clarifies it perfectly. We are filtering the current intent, not the person’s entire identity Exactly And once we get a good click, we have to track every single conversion, right?
Every form fill, every phone call, and feed that tracking data back to Google This is how you train the machine. When you report back to Google that a specific click resulted in a booked appointment, Google’s machine learning analyzes the thousands of data points on that user’s profile Wow, thousands of data points Oh, yeah.
It looks at their browsing habits, their location, their demographic markers, and then it goes out to find more people just like them. If you don’t feed the data back, Google is just operating in the dark Right When you [00:08:00] provide that feedback loop, you basically turn Google into a highly trained scout hunting exclusively for your ideal patients That is incredibly powerful.
So paid ads get the phone ringing today, but obviously you can’t rely entirely on a pay-to-play model forever because the moment your credit card declines or you turn the campaign off, you completely disappear Right. You need long-term assets Right. To build an asset that actually appreciates over time, you have to establish digital trust.
And for a local clinic- That trust is almost entirely governed by the map pack The holy grail of local search Yeah, that coveted box with the map and the top three clinic listings- Yeah … right below the ads. But so many chiropractors treat their Google business profile like a static Yellow Pages ad from 1998.
They really do. They claim it, put in their hours, and literally never touch it again. Right. The source material emphasizes that this is a massive mistake. The map listing is now a living, breathing social [00:09:00] profile. It requires constant feeding They suggest treating it almost like your Instagram feed. Like you should be uploading new photos of the clinic, short videos explaining conditions, and special offers at least once a week.
It signals to Google and to the prospective patient that the lights are on, the practice is thriving, and there is active energy in the building One of the most fascinating strategies they discussed was the 360 degree virtual tour. Oh, that is such a cool tactic. It is. You hire a local photographer to come in with a specialized camera, and they stitch together a walkthrough of your office.
A patient can click, drag their mouse, and virtually walk through your front doors. They see the waiting room. They see the adjusting bays. The psychology behind this is just brilliant. Think about the barrier to entry for a new patient who has never been to a chiropractor They’re usually pretty nervous Exactly.
They are in pain, and they are probably carrying a lot of anxiety about getting cracked. Allowing them to virtually explore the space lowers their nervous [00:10:00] system response before they ever even leave their house They feel subconsciously familiar with your clinic Yes. And on the technical side, because they are spending 45 seconds dragging their mouse around your virtual tour, their time on page skyrockets Oh, and Google loves that Google measures all that engagement.
Higher engagement scores literally force your ranking higher in the map pack That is a massive return on investment for a simple one-time photography fee Huge But we really can’t talk about the map pack without talking about the undeniable king of local rankings, which is reviews. Nick Fischer pointed out that it’s not just about the total number of reviews, um, it is about review velocity Because consistency is everything to the algorithm.
If practice A has 200 reviews, but the last one was from three years ago, and practice B has 50 reviews, but they are consistently getting three new ones every single week, Google will heavily favor practice B That makes sense. A sudden drop-off in reviews makes [00:11:00] Google’s algorithm wonder if you closed down or changed ownership or maybe if your quality of care plummeted Right.
You need a steady ongoing stream Which obviously requires systems. You can’t just hope patients remember to do it when they get home Never works No. The source recommends physical tap to review stands sitting right on the front desk paired with automated text messages that trigger right after their visit But a participant in the Zoom session asked a really practical question.
Oh, I remember this. Yeah. They said, “If a patient already left us a glowing five-star Google review last year, can I ask them for another one when they return for a new injury?” And the technical reality there is that Google only allows one review per user account for a specific business. You just cannot stack multiple Google reviews from the same person.
Right. But this is actually a massive strategic opportunity. If that loyal patient wants to sing your praises again, you redirect them. You ask them to drop that second review on Yelp or Apple Maps or your Facebook page. That is smart because it creates cross-platform [00:12:00] consistency. The AI platforms and search engines crawl the entire web, not just Google.
Exactly. If you are highly rated on Google but completely invisible on Apple Maps, it looks unnatural. It looks slightly inconsistent to the bots. Right. When you have glowing reviews everywhere, your overall digital trust score skyrockets. And speaking of reducing friction in the map pack, there is an urgent technological shift happening right now.
You absolutely must connect your scheduling software systems like ReviewWave or our Engage software directly to your Google Map listing. Yes. Patients can now see live availability and book organically right from the search page without even clicking over to your website. Think about the modern consumer.
They want immediate gratification. Mm-hmm. If you make them click three times just to find a phone number, well, you’re losing them to the clinic down the street that lets them book a 3:00 PM slot with one tap on the map. It is the ultimate reduction of friction. Now, the map pack captures the highly motivated patients who are ready [00:13:00] to book right then, but what about the other 90% of the population?
That’s a huge number. Nick Fischer shared a stat that only 10% of Americans see a chiropractor annually. The other 90% aren’t typing chiropractic care near me. They don’t even know you are the solution to their problem. They are searching for their symptoms. They are typing like how to get rid of a tension headache or sharp pain down my leg or, you know, what to do for neck stiffness after a car accident.
Exactly. And this leads us to the third tier of the search page, which is organic website architecture. We really need to dismantle what the source calls the homepage trap. So many doctors think their beautifully designed homepage is going to catch every search. But it just doesn’t. No, it doesn’t. If a patient is searching for sciatica relief and your homepage only talks broadly about subluxation, family wellness, and your clinic’s history, Google is not going to show your site to them.
Right. To rank organically for symptoms, your website needs individual deep pages for each specific condition and service you treat. And we [00:14:00] are talking robust pages. Mm-hmm. Around 1000 words each dedicated entirely to headaches or entirely to neuropathy or entirely to auto accidents. Yes, deep content. And these cannot just be massive boring walls of text.
These pages must feature FAQs, comparison tables showing the chiropractic route versus the traditional medical route, and clear irresistible offers. You drop $127 initial visit special right in the middle of the text. Exactly. Immediately followed by a Book Now widget. You educate them on the exact pain they are feeling- Mm-hmm
and then you give them an immediate off-ramp to actually become a patient. To boost the authority of those specific pages, the source recommends building a local resources page. This is basically a networking strategy applied to digital SEO. Right. You swap backlinks with other local health and wellness businesses.
So like a local CrossFit gym, a massage therapist, maybe a holistic nutritionist. You link to their sites, they link to yours. That proves to [00:15:00] Google that you are a deeply embedded trusted part of the local community network. Exactly. But I have to push back here for a second because they introduced this concept of EAT, right?
Experience, authority, and trust. Yes, EAT. And they talked about building out deep bios and content to prove this authority, and I’m putting myself in the shoes of a local doc listening to this. I’m thinking, “I am a chiropractor in a small town. I am not a global influencer like Gary Vee. Why on earth do I need to host a podcast or have a massive bio online?
I just want to adjust spines.” It is a completely fair pushback, but it ignores a fundamental shift in how search engines actually view the world today. Google and increasingly AI engines draw a hard line between a brand and a human. Okay, what do you mean? A clinic website is just a brand. Anyone can throw up a WordPress site and claim they cure back pain.
To rank at the top, the search engine needs to mathematically verify that the medical information is coming from a trusted, authoritative human expert. [00:16:00] That is where EAT comes in. Hold on. Mathematically verify? How does a local doc prove they are a human expert to a machine? Through a deeply structured bio page that lists your credentials, your associations, your awards, and your publications.
But here is the key. This information needs to be coded into the back end of your website using something called Schema or JSON code. Okay, stop right there. JSON code. I’m a chiropractor, not a Silicon Valley developer. You don’t need a computer science degree, I promise. Think of Schema simply as a digital business card formatted perfectly for Google’s crawlers.
Okay. Instead of Google trying to read your website’s paragraphs to sort of guess if you are a doctor, Schema hands them the data directly in their native language. It literally says, “Here is Dr. Smith. Here is his medical degree. Here is his license number.” Oh, that’s actually really straightforward. It is.
Most website platforms have simple plugins that generate this code for you automatically. Furthermore, when you host a short local podcast or [00:17:00] put a video interview of a successful decompression patient on YouTube, you are creating a web of credibility. You aren’t just a faceless clinic. You are Dr. Smith, the recognized verified authority on spinal health in your county.
And that human authority acts as an anchor that lifts the ranking of your entire clinic’s website. Proving the human authority is vital because the entire landscape of search is changing. As the Zoom session heavily emphasized, we are rapidly entering the AI search revolution. It’s moving fast. It really is.
The search engine is evolving from an answer engine that just gives you a list of blue links to click into an action engine. And this is the frontier that every practice owner needs to prepare for. Soon, a patient won’t type into a search bar. They will just tell the AI assistant on their phone, “I have severe lower back pain.
Find me someone who can fix it today.” Wow. The AI won’t just list websites. It will evaluate the local clinics, check your live calendar, check your transparent pricing, and say to the user, “Dr. Smith has [00:18:00] fantastic reviews, treats back pain, charges $127 for an initial visit, and has an opening at 3:00 PM. Shall I book it for you?”
And the AI will literally book the appointment on their behalf. Think about how AI works It wants to take action. If your calendar isn’t live, well, the AI can’t book it. If your prices aren’t listed transparently, the AI can’t quote it. It doesn’t just rank you lower, it skips you entirely and books the patient with your competitor who is digitally integrated.
Exactly. And formatting your content for AI requires a completely different approach. AI does not want to read a meandering poetic novel about the history of chiropractic. Nobody does, right? True. But AI specifically wants content broken down into TL;DR, too long; didn’t read summaries at the very top of the page.
It wants myth versus fact tables. It wants quick how-to steps. Just giving it the data. Right. AI wants short, punchy answers early in a paragraph so it can easily scrape that data, feed it to the user, and cite your clinic as the source. Okay. Pause. I’m listening [00:19:00] to this right now. I’m seeing 80 patients today.
My main CA just called out sick, and you’re telling me I need to write 1,000-word essays on sciatica, format tables for AI, shoot videos, and generate schema code. I mean, I’m tapping out. That sounds like a recipe for complete burnout. It absolutely would be if you were doing it one piece at a time. This is where Nick Fischer’s concept of the content distribution multiplier saves your sanity.
Okay, tell me about the multiplier. You do not need to create 100 separate pieces of content. It is exactly like taking one set of X-rays. You take the X-ray once, but you use it to explain the adjustment to the patient, you use it to file the insurance claim, and you use it months later to show the patient their structural progress.
One asset, multiple returns. That makes perfect sense. Yeah. So how does that apply to the content? You sit down and shoot one five-minute video explaining sciatica. That is the X-ray. You upload that video to YouTube. You embed that YouTube video onto your sciatica symptom page on your [00:20:00] website to increase the time on page metric.
Brilliant. Then you strip the audio and use it as a podcast episode. You hand it to a virtual assistant to cut into short clips for Facebook and Instagram, and you post it to your Google Map listing. You did the heavy lifting once for five minutes, but the internet sees you everywhere. Consistency with this multiplier over time builds an unstoppable compounding effect.
Exactly. You do the work once, and the ecosystem goes to work for you. And if you pair that with the new AI calling systems they mentioned, like technology that can act as a virtual assistant to answer your clinic phones after hours, quote “pricing,” and book the patient while you are sleeping No lead is ever missed just because it’s Saturday morning.
It is a lot of strategy to absorb, but it all anchors back to one core concept: building a digital asset that unequivocally proves you are the most relevant, prominent, and trusted solution in your community. So I want to leave you with this final provocative thought to mull over today. Let’s hear it.
Think about your digital footprint [00:21:00] right now, in this exact moment. If an AI assistant was forced to evaluate your online presence today, your review velocity, your website depth, your verified human authority, to decide if you were the safest, most trusted doctor in town, what would it conclude? Would it book the appointment with you, or would it automatically send that patient to your competitor down the street?
That is a heavy question, but answering it honestly really defines the future of your practice’s growth.
